Cisco Catalyst 3020 in HP blade center chassis not working

I have a Cisco Catalyst 3020 switch installed in IB3 of an HP c3000 chassis.  I have uplinks working to the upstream switch, but the internal ports to the blades show as down.  We have a BL460c G8 blade server in slot 4 of the chassis and it has an HP module in Mezz 2 to provide Ethernet interfaces.  We can see the Mezz 2 interfaces on the server in the HP Chassis Management console and in the OS for the server but they show as disconnected there and they show as down in the Cisco 3020.  They should be ports 4 and 12 based on the documentation I have seen.

 

Does anyone have any further experiences/insight on this and what may need to be done to fix or at least troubleshoot this?

Did you get chance to check on the physical layer issue on the interfaces on switch 3020 ,can you validate the speed and duplex setting on the interfaces.try to configure it manually,as full duplex  and speed 10/100/1000.See if interface comes up .
