Deployment HyperFlex and process of HyperFlex Cluster Error

On the process of HX Installer ,config the all parameter(server FI IP account Vlan and so on) and then deployed last step progress(Complete HX Data Platform Deployment、HyperFlex Cluster  ) It occur error! As follow:

Installing Software Packages on Storage Controller VM
Checking and Clearing Disk information if requested. failed with 'Disk /dev/sdd is already formatted. Select Clean up disk partitions under Advanced to format all disks.'

 HX installer version is:2.1(1b)
             vcenter:6.0.0U3b-5326177
               exsi:HX-Vmware-ESXi-60U3-5050593-Cisco-Custom-6.0.3.1-HX
      ucs manager:3.1(2g)
        ucs firmware:3.1(2g)a  b  c

Please back to the settings and from Storage configuration select Advanced Configurations and Clean up disk partitions

This is a VMware issue. Disconnect the hosts from vCenter and reconnect them.

Important:Do not remove the node from the cluster. This is a disconnect only.

During HX Data Platform deploy, the IP addresses page lists the same servers twice.

During HX Data Platform deploy, the IP addresses page lists the same servers twice.
Action: Select only one from pair
This might occur when UCS Manager configuration is skipped and HX Data Platform references both UCS Manager and the imported JSON file. Select only one of each pair of IP addresses.

Action: Disconnect and reconnect
This is a VMware issue. Disconnect the hosts from vCenter and reconnect them.
