Nexus 7000 module will not power on

We have a spare N7K-F132XP-15 module that had not been used in some time, and when trying now it will simply not power on when in a C7004 chassis, in either slot of the chassis.  I also have an N7K-M148GS-11L in the same chassis that works fine in either slot.  The 'show module' command displays:

I noticed that the model number of mod 4 is not updating to display that from the F1 instead of the M1, even though the model type value is updated.  I also have a 7010 chassis that I put the F1 into, and there it worked just fine.  Both the 7004 and 7010 chassis are running the same software version 7.2(1)D1(1).

1. If the problem is with the 7004 chassis, why is the M1 card working fine?  
2. If the problem is with the F1 card, why does that card work fine in the 7010?  
3. Does anyone have any ideas of further troubleshooting things to try?
