Whatever I put for the Radius server ip the switch always show a log message with wrong server ip 0.0.0.0. Checking the log I can see this message.
Debug message: "Radius server is set to ip 0.0.0.0 port 1812 key 'testing123' priority 65535 timeout 10"
The Radius ip I entered was 10.0.0.1
The switch is on the latest firmware 1.2.1.5
Any idea how to fix this problem.
Thanks.

- How do you specify the radius server ip ?

I ran command
radius-server host 10.0.0.1

- Being a sanity check only try some address without zero's and check if then the correct
not working address is seen in the logs , (as you specified it)

I tried
radius-server host 10.2.3.1
Same issue.

- Strange , you could save the configuration of the SG , factory reset it , restore config and check
if it happens again , (or before restoring the config check if it happens
on a virgin device too) yet this argument is rather weak I must admit and I have no further/other clues
for the time being. You can or could contact Cisco ; check link below.

I ran data packet analyzer on one port to see if the switch is sending authentication frames by any chance despite the wrong ip in the debug log. There is no packets being sent from the switch.
Looks like a bug in the firmware.

- Yeah , looks like a bug ; you could try the CLI command show radius-server ,
to check if any radius server has been defined after you try the command and what
the reported ip address is (e.g.)
