SG300-52P How to setup Two Vlans voice and data and have cisco 7945G pull proper Vlan

These switches go over a VPN back to main campus for Cisco unity support.

Phones do not pull proper VLan.  Cisco support tells me I need to configure the phones to know vlan.  Switch cdp or lldp should be doing this

Can you post a sample configuration of one switch port?

also how does it know how to router to that vlan 5 interface , seems to me the switch has to have a ip bound somewhere , ?
