I have kind of a weird issue going on. I currently have an 1921 with a GLC-LH-SM that reaches out to another building and currently working great.
I'm trying to migrate connections over to a 4331, but whenever I insert a GLC-LH-SM transceiver into g0/0/2 (the dedicated SFP) I get an error stating %TRANSCEIVER-3-NOT_SUPPORTED: SIP0: iomd: Detected for transceiver module in Gigabitethernet0/0/2, module disabled.
I ran the service unsupported-transceiver command, and am now receiving the error %TRANSCEIVER-3-NOT_COMPATIBLE: SIP0: iomd: Detected for transceiver module in Gigabitethernet0/0/2, module disabled.
This is a Cisco branded gbic, and I am on the 3.16.02S code and the transceiver shows that it is on the supported list - so I don't know what is going on. Has anyone else had this problem on this router?

Yeah, I have the same problem with a 4331 router and a glc-sx-mmd running 3.16.02 IOS.
My 4331 does not complain about it being not_compatible, usually that happens only when you run a old IOS version..
funny thing is that its working on another 4351 router that I have, same IOS version, same SFP type.
I am currently creating a TAC case via my Cisco partner, so hopefully within a few days I will have a fix to the problem.. I will let you know what the outcome was..

I managed to establish the reason for my problem, but I'm still unsure of the root cause. So, I tried a newer (3 years old) GLC-SX-MM in the same SFP port, and my original 4431 Router immediately recognises the adapter. When I use an older (6 years old) GLC-SX-MM in the same SFP port, my 4431 Router does not recognise it, but the older adapter works OK on an older Router (3845). Therefore there must be a difference between older and newer adaptors of the same type....in that my Cisco 4431 only likes the newer GLC-SX-MM. Cisco TAC have yet to explain to me the difference between the older and newer GLC-SX-MM, or to identify why this occurs, but at least I have a solution by using the newer GLC-SX-MM. So, in your case a brand new GLC-SX-MM might help.

Im using a brand new GLC-SX-MMD module, so slightly different..
The router actually sees the module correctly in the Ge0/0/0 slot, but not in the Ge0/0/2 slot?!?
TAC just informed me that they are going to RMA the router, not sure if its going to work, but whatever they think is best.. ;-)
I will let you all know how it goes.. Perhaps if I find time, I will try and setup another 4331 router from stock and see if that works, but I am a little pressed on time right now.

software change fixed it for us , zx model sfps , glcts and glxsx had issues with all of them on different software typesnonly some would work on certain versions , the one software version they all worked on was .....asr1001x-universalk9.03.16.04b.S.155-3.S4b-ext.SPA.bin

Try command:
service unsupported-transceiver
Remove the optic and re-install.
This worked for me onisr4400-universalk9.03.13.04.S.154-3.S4-ext.SPA.bin
Before command:
*Aug 2 21:53:16.160: %TRANSCEIVER-3-NOT_SUPPORTED:iomd: Detected for transceiver module in Gigabitethernet0/0/3,
*Aug 2 21:57:54.349: %TRANSCEIVER-6-REMOVED:iomd: Transceiver module removed from Gigabitethernet0/0/3
After command:
*Aug 2 21:58:12.341: %TRANSCEIVER-6-INSERTED:iomd: transceiver module inserted in Gigabitethernet0/0/3
Interfce cam up and is working fine.

I have read through all the post, but it seems like there is still no clear solution.
Does anyone know if GLC-LH-SMD SFP is not supported on GE0/0/2 port of ISR4331/K9?
It sounds like only the GLC-TE (RJ-45) SFP is supported on GE0/0/2 port of ISR4331/K9.

Is there any one resolved this issue ? our isr4331 Gi0/0/2 port still down status, it also insert GLC-SX-MMD and replace it multi times...
