cisco switch getting server doesn't respond when signing on with GUI

Trying to sign on the web interface of switch.  It's gets to sign on page than it times out saying server doesn't respond.  

When I connect with SSH, the message %AAA-W-reject message appear.  Have tried most on the web.  Some of switch can logon without problem

ip http authentication aaa login-authentication radius LOCAL  <<- config local as fallback 
aaa authentication login SSH radius LOCAL <<- also here you dont use Method default you use SSH, so you need to re-config the auth under the vty 
aaa authentication enable SSH radius LOCAL
aaa authentication login Console radius  LOCAL <<- I Prefer use LOCAL for console not use radius server, it last resort if we lost connection to radius 
aaa authentication enable Console radius LOCAL
aaa authentication enable authorization default none <<- what is this command !!

since we config LOCAL and to prevent any issue use following THIS IS SO IMPORTANT 
username xxxx password xxxx
username xxxx privilege 15
enable password xxxx
