I have a printer that will appear for 3-5 pings after the port is bounced, then it disappears. I have removed all port security config from the port, upgraded the switch to newer iOS, and rebooted it. Printer works if we remove the phone from the equation. Port was never in err-disabled state (always up and up). No port security violations. Cables tested good.
What is the next step in troubleshooting this problem? I do not have physical access to the access switch or printer.

Can you hard set the port and the printer's NIC and test again?

Sorry for the delay - I do not have physical access to the printer and phone, so it took some time to get someone over there.
Hard setting the printer's NIC did not work. Both the port and the printer are now set to 100/Full. The phone came up but the printer never did. Now when I bounce the port I do not receive the 3 to 5 successful pings.
