PLease take a look at the sh version snippet.
1)What is the frequency of a IOS version being in this format? Particularly the second number in brackets (18a)

I'm not sure to have understood,
This is an IOS image that runs on a RSM for Catalyst 5000.
IOS version is 12.0(10)W5
the final (18a) means it is a rebuild the 18th rebuild with a sub-rebuild a.
This is typical of end of sales devices where the policy is to freeze the major version and to make only minor changes rebuilds as needed.
