Hi
I have a Cisco WS-C3560E-12D-E switch, which tells us in its log about a faulty fan.
Strangely enough, the sho env all command returns an OK for all four fans, but in the overview (last two lines) it declares Failure-Fan on both power supplies.

FAN 1 is OK

FAN PS-2 is OK
TEMPERATURE is OK
Temperature Value: 28 Degree Celsius
Temperature State: GREEN
Yellow Threshold : 37 Degree Celsius
Red Threshold : 52 Degree Celsius
SW PID Serial# Status Sys Pwr PoE Pwr Watts

Does this indicate a problem or was this just a momentary hiccup? I'm a bit concerned.

Silly me, should've thought of the Software Bug Toolkit!
We still use version Version 12.2(50)SE2. We'll upgrade to a more recent version, that will resolve the matter.
