Is this h/w issue ??

The supervisor card is dead. You may need to create a TAC Case using this thread.

Hi,
you can try do hard reset (remove the card and insert back again) . sometime sup card loose connection with back fabric will cause this issue. So mostly resetting card will help the sup to boot.
If the sup not recover even after resetting then most probably a hardware issue .

Same issue happened many time in our environment for 6500 series switches and solution is to hard/soft reset the module.
If problem persists, problem with sup-engine
