WS-X6148A-45AF coming up as unknown

We have a 6509-E with SUP720-3B running 12.2(18)SXD7b and the above module is not coming on-line. Below is a show module and the result if when the module was inserted into slot 9

I have researched on CCO against the hardware, and the suggested minimum software is the one we are running and also 12.2(17d)SXB11a. We have tried the second code and get the same results.

Has anyone else experienced problems with this card, or know of a resolution?

What power supplies are you running. Can you post the output of a "sh power".

It may well be you do not have enough power to power up that card and the 6500 will start at slot 9 and work backwards in terms of modules it disables.

Thanks for the replay. PSUs are 6000W and we know that we have enough power. Just found another reference on CCO that states we need 12.2(18)SXF, so that conflicts with what we saw yesterday! :(

Just downloading the latest version, and will see how it goes.
