On the sx20 camera extension using 2 of the CAB-PHD4XS2-SPLI is the cat 5 a true cross over cable (wo, o, wg, b, wb, g, wb, b to wg, g, wo, b, wb, o, wb, b) or is it ONLY pin 3 and 6 flipped?

You're going to need to make a custom cable most likey, as the pins don't line up per a straight-through or corssover cable, refer to the pin-out of the RJ45 female end of the CAB-PHD4XS2-SPLIT in the PrecisionHD Camera User Guide on pg 20.

If you only use one of the CAB-PHD4X2S-SPLIT cables and the CAB-HDMI-PHD12XS cable (Option 1), you can use a straight through cable.
If you're using two of the CAB-PHD4X2S-SPLIT cables (Option 2), you need to use a custom pinout.
See https://supportforums.cisco.com/sites/default/files/legacy/3/2/2/15364223-Extending_PHD_4xS2_Camera.pdf for description and pictures of both options.
For either solution, you will need to power the camera separately with a PSU-CAM-V (or equivalent).
