Cisco Unity Connection E164 number falls to failsafe when accepting recordings

If the CUCM Call Manager is using E164 numbers and the same is imported in CUC, messages are accepted which are coming over the PSTN and distant SIP Trunks, but from the same location, other extensions while leaving voicemail, the system replies " The system is temporarily unavailable to accept the call ", When you do port monitoring, it says "Failsafe" .

 

Any idea, what i am missing? The Extension in the company are E164 and the same imported in CUC. and want to resolve the issue.
