Cobras Import for Unity Connection

Im trying to run a COBRAS Import for Unity Connection.  I would like to import only the distribution lists which I pulled from the Subscriber Information dump however when I select "import from CSV file" I get the error that the data can not be read properly.

What is the header for the Public_DL that COBRAS import is looking for and does any other headers need to be present?
