VM for forwarded calls Unity

I have a Unity 4.0 system integrated with Exchange. Leaving and retrieving messages works fine when I call a phone which is forwarded to VM.

The problem occurs when I forward one IP phone to another IP phone which is finally forwarded to Unity VM.

Then I get an error message "A system error occured. Please wait for an operator".

Any ideas?

You should get something in the application event viewer logs. Also, what is the exact version of Unity and the TSP (You can use the Gather Unity System Info tool from the Tools Depot to find this).
