B200 M4 can not boot from SAN

SIMMN

I have two new M4 (spare actually) installed in a chassis. The firmware has been auto updated to match the Chassis and FI (version 3.2(3g)). The SAN admin helped me create boot luns for the two blades and I finished installation of ESXi 6.0. However the issue is the blades can not boot up after the ESXi installation. KVM shows screenshots below.

The boot policy I have is legacy mode, not Uefi. The service profiles was generated from existing template and which is pretty much the same as another 8 blades in the other chassis managed by the same FI pair, Those blades are fine booting from SAN with ESXi 6.0.

Please advise.

Hello,

Would it be possible to provide a screenshot of the boot policy for the servers?

Also if this generates any output, provide it as well.

connect adapter [chassis #]/[blade #]/1
connect
attach-fls
lunlist
