The connecting port at the switch and all interfaces in the AP need to be trunk ports and the native VLAN between the switch and AP must be the same. All you need is a helper address on your SVI interface for DHCP.
We experienced the exact same issue when we moved from our 3500s to 3750s. The issue ended up being a TCP port I was filtering through a VLAN filter. If you have any filters/ACLs applied try removing those temporarily and see what the result is.
I've had the same issue with our HQ VPN to foreign office, with the HQ VPN DF flag set like you described. If the web server is MS IIS try appling MS black hole detect reg edit. This resolved our issue.
