My Duo admin log just shows 0.0.0.0 for Access Device, and “Location Unknown”. Second factor usually shows a location which I’d assume might be based on a cell tower location, if the 2F is from a phone. My own, for example, is shown as my next town....

One thing you might try is to run the authproxy_connectivity_tool.exe located on your authproxy server in C:\Program Files\Duo Security Authentication Proxy\bin folder. This should verify connectivity between DUO / AD and the authproxy.
