Yes the Stack is connected only with one Cable to the 2960x Distribution switchThe Distribution 2960 STP config is rstp and priority is set to 8194 so he is the root bridgeAll the switches have the Image 15.2.4E6 which is the recommended version.

Exactly same issue here.I don't know if I should downgrade or upgrade the Switches.It is happen to Singleswitches and Stacks.I have open up one TAC case and hope to receive Answer quickly because this issue block my schedule.
