SG350-28MP Switch Random Reboots: Help!

I have a 28 port Gigabit PoE Switch here that is doing constant, unscheduled reboots; about once every 2 weeks.

 

This is now the 2nd switch we are on. I already RMA'd the first one because of the same issue.

 

Native/untagged VLAN for generic internet traffice and a Voice VLAN for VoIP traffic. LLDP is enabled. CDP is also enabled.

Mainly running old Polycom Soundpoint 335 VoIP phones, that do not take up much resources and power.

 

Yet for whatever reason it reboots on it's own.

 

Environment is good: Temp and Fans. Albeit that thru the 'show log file', I was getting a pretty decent amount of [:%Environment-A-FAN-STAT-CHNG: FAN# 2 status changed to not operational.] 

 

Is there anything I am just plain missing that might be causing issues with random reboots? I am still very green with Cisco switches. Is it appropriate to be running LLDP and CDP simultaneously?

 

Any help would be golden!
