UC540 MWI not working after changing IP address of Service Module

This is the second UC540 I've deployed and know it well however; after changing the integrated service module from the standard IP, the MWI stopped functioning.  Everything else is functioning just fine.

 

Going through the debugs, below is what I've noticed.  I know there are some obvious errors but all settings should be correct as the only thing that changes is the module IP.  Any ideas?
