We have a 1 x 2008r2 server running radius; straight forward setup via Cisco documentation. The Trap logs are full of these about every 10 minutes. I've confirmed the SECRET between the Cisco WC & Radius are correct and users can connect....most of the time.
What I've done thus far....
From Wireless Controller:
RADIUS server 10.10.10.225:1813 failed to respond to request (ID 137) for client 14:7d:c5:c2:b8:78 / user jsmith
RADIUS auth-server 10.10.10.225:1812 available
RADIUS auth-server 10.10.10.225:1812 unavailable

Did you get this resolved? I have the same problem on a 7500 flex controller.

There is most likely still lacking configuration on your Windows Server 2008R2
Personally, i dont like using Windows Serv as a straightdown RADIUS server. Configuring RADIUS on windows can get quite complicated
Most of my deployments use:
1. LDAP -Polling AD Users
2. Cisco ACS -RADIUS and TACACS+
Also, have you tried disabling firewall on your server? If you have any AV with firewall software on that, make sure to disable that too.
Additionally, looks like your RADIUS connection is not reliable, considering it becomes available, then becomes unavailable again vice-versa i presume
Better verify that your RADIUS timers on your server matches that on the 2504
