CWMS 4.0 not syncing to CallManager LDAP _ Directory Integration

We were recently trying to upgrade our CallManager publisher from 11.5 to 12.5. During the upgrade process, the CallManager Publisher (on ESXi) crashed and we had to rebuild it from DRS. Ever since the CallManager Publisher was rebuilt the LDAP Directory Integration can no longer succesfully sync with CallManager.
After we save the credentials on the Directory Integration page in CWMS the system says "Testing connection to CUCM..." and then is followed by "CUCM unreachable"
After we try the actual LDAP sync, CWMS returns this message: "Network Failure. Retrying synchronization."
No matter which of our existing username/password combinations we use CUCM remains unreachable. We can ping CallManager from CWMS and user are able to start/create messages from the Outlook and WebEx plugins without issue. The only problem we see is that CWMS can't sync with CallManager. Users don't seem to be affected.
And this is what we're seeing in the CWMS logs:

Have you figure this out? im also facing the same problem between CWMS 4 MR2 and CUCM 12.5 SU3
