The service port number is not setting correctly. The number should be 8080.

To resolve this issue, replace the existing C:\Program files\wfaavvid\tomcat\webapps\ROOT\form.jsp file with a newer downloaded form.jsp.

This new form hard sets the service port number to 8080.
