Agents stuck in "Offering" state/Ready State same time

Hello Everyone,

I am running into an issue which is weird. I hope someone out there knows how to fix this:

Customer calls from outside. This incoming call hits the Queue. Agent "A" picks up the call. Agent "A" transfers this call from his IPCC extension to Agent "B" on his private extension. Agent "B" does not pick up the call and lets the call go to his Voice Mail. Once the call goes to Voice Mail, now Agent B is stuck in "Offering" state and in "Ready" state. Agent "B" screen at the top shows "Offering" while at the bottom shows "Ready". Agent B is able to get the calls in queue but this one missed call always stays in CAD as "Offering" and does not get cleared until Agent exists CAD ang logs back in.

Any ideas on this. I have TAC case open and was told to apply the patch. I have applied the patch but no resolution.
