I follow the procedure by the book and I doesn't work ? What have you done to make it work ?
FYI: I' m on CCM 4.1 co-resident with IPCX 4 and the directory is AD 2003.
What is your tricks ?

Did u install AD plug-in from Call Manager...

I have Co-resident too, but there is AD plug-in in CCM,
-On Domain Controller allow SCHEMA UPDATE, BY CHANGING (HKEY_LOCAL_MACHINE\SYSTEM\CurrentControlSet\Services\NTDS\Parameters\Schema Update Allowed) and verify that the value is set to 1
-Create OU in your AD!
-Then U have to install plug-in, to integrate CCM into AD,
-Change regedit key in HKEY_LOCAL_MACHINE\Software\Cisco Systems, Inc.\Directory Configuration
-In the right pane, double-click the DirAccess registry key (DIRACCESS).
-Delete the false registry entry and enter true as the new registry entry.
-Restart IIS
-With CCMPWDChanger from cmd change passwords for CCM system users
-Browse to C:\dcdsrvr\config, and open the file, UMDirectoryConfiguration.ini, in Notepad.
-In the UMDirectoryConfiguration.ini file, locate the key, UserDirAccess.
-Change the UserDirectAccess value to true.
-In Notepad, choose File > Save to save the file.
-Restart IIS again
-Next within CCM-Users-Access rights-MLA- change value from false to true and insert password for CCMadministrator
So u can add users from CCM to AD
Then, go to CRS Admin page for 1.st time it should give you Directory setup, AD info and just change parts that you configured into CCM and AD and that should be IT
