7916 Expansion Modules frequently reset

I have a 7975 phone running load SCCP75.9-4-2SR1-1S. The phone has two 7916 expansion modules attached as per the related documentation.  Both expansion modules frequently reset/reload for no apparent reason.  When this happens it has no impact on the phone itself.  Everything that can be replaced has been swapped out, the phone, both expansion modules, the power brick, network jack, cabling, and switchport.  I'm at a loss on what to try next.

For the record, following are the versions of everything:

Cisco UC CM: 10.5.2.12901-1

7975 Phone Load: SCCP75.9-4-2SR1-1S

7916 Modules: B016-1-0-4-2

If anyone has any thoughts on how I can resolve this please let me know.

Mike,

Please refer this Bug below. Try to downgrade firmware version of 7975.. I tried with this 9-3-1SR4-1S  & works.

Symptom:
7962 or 7975 SCCP IP phones with side cars may reset.

Conditions:
7962 or 7975 SCCP IP phones with side cars and BLF configured on the side car is activated by calling the phone that is being monitored by the BLF line.

Workaround:
None

Further Problem Description:

Thanks very much for your reply.  I will give this a try although it will probably be tomorrow before I can.  I will reply with results.

Is the phone connect to the wireless headset?  If yes, try to disconnect them.
