I've searched high and low on the internet and cisco, and not finding answer for what these console messages on a 8865 IP Phone console log mean prior to the DEAUTH. I've stumbled across some hits on linux and android boards but not finding the reason code numbers or etype numbers yet.
This is on a 8865 IP phone with 11.0.1 firmware.
What is wlanmgr(309)-wm_event_callback etype:5 ? There are a few dozen of these messages in log prior to the DEAUTH.

Can the phone connect to an SSID with open authentication?

I'm having the same issue with an 8861 desk phone. The phone connects and works fine but every now and then the phone will unregistered and we have to reset all settings on the phone and reconfigure the wireless settings.
