I have C2911 with loading c2900-universalk9_npe-mz.SPA.152-4.M5.bin
(but with previous releases situation was same )
I use ССM Version 9.1. in my bundle. I have native phones (6921, 7962) and several SIP DLink gateways. ISP interconnection by SIP.
I have IVR on it, but when I stoped IVR - situation was same.
In random time moment
CPU utilization for five seconds: 99%/0%; one minute: 99%; five minutes: 99%

177 2034368 15814 128643 98.61% 97.98% 97.89% 0 AFW_application_
and all calls rejected - AFW_application hungs up.

and what's worse: no fix (apart from downgrade?)

Problem was found. There was one loop in voice-hunt group. Last number point to pilot number.
So after some loops - 100% AFW_application_

Had the exact scenario with AFW_application_ eating 100% CPU. Checked my hunt groups and I had a loop.
voice hunt-group 1 parallel
phone-display
list 3400,3401,3402,3403
timeout 30
pilot 3400
