Blank Service on Cisco Ip Phone 7940

We got a problem, which our system is using CUCM 6.1.4,

and we using 7940 cisco ip phone with firmware sccp 8.1.2,

we also using EM for login and logout for user, and the problem is, when the user try to logout after login to the ip phone, the user got a blank service shown,

we already subscribed the needed phone services for logout on user Device Profile, and for information not all ip phone 7940 is can't logout, it's only a 5-7 ip phone 7940,

actually it's solved by restarting the ip phone, and the logout service showing again after the restart, and can login or logoff

we also captured the debug ip phone logs, the logs written like this,

is this the 7940 firmware issue? or it's bug on firmware SCCP 8.1.2,

do we need to upgrade the firmware?
