I cant remember what causes this, but I have a cucm 6.1 with 7961 phones. New out of the box, they start the upgrade process to my 6.1 and then come back up with the bullseye looking graphic in the lower corner of the screen. (normally see the check mark box)
how do I get rid of that again?

7970 becomes inoperable after firmware upgrade attempt
Symptoms:
A 7970 IP Phone becomes inoperable after a software upgrade. The Cisco logo
appears on the screen with a Checkbox in the bottom left corner and the
Line LED's blink from top to bottom in green. The phone does not respond to
any user input.
Conditions:
May occur under rare conditions after a software upgrade.
Workaround:
None. To recover from this situation, a reset to factory settings may fix the
problem.
To reset the phone do the following:
Note that the phone must be on a network providing an IP address and TFTP server
address using DHCP and the phone must be powered via external power brick. The
Alternate TFTP setting is erased when using this procedure requiring that the
TFTP server address be provided by a DHCP server to get the phone upgraded.
1) Power off the phone
2) Hold down the '#'.
3) While holding '#', connect an AC adaptor to the phone.
4) Continue to hold '#' until the line buttons blink amber.
5) Release the '#' key.
6) Enter: 1 2 3 4 5 6 7 8 9 * 0 #
7) The lights should change color from amber to red. The phone is now resetting
to factory defaults.
If the above does not work, try the following:
1) Power off the phone
2) Hold down the '#'.
3) While holding '#', connect an AC adaptor to the phone.
4) Continue to hold '#' until the line buttons blink amber.
5) Release the '#' key.
6) Enter: 3 4 9 1 6 7 2 8 5 0 * #
7) The lights should change color from amber to red. The phone is now resetting
to factory defaults

i wish it was that easy. Its on CUCM 6.1. I now remember I had this problem with the TFTP server dropping packets. The NIC card was mismatched with the port. As soon as I fixed the NIC card, the TFTP server started to work fine and phone loads worked.
Im still not sure what the technical term for "bullseye" in to search for.

We had this same issue today as well. In our case, the cable drop apparently has issues. I'm suspecting either a pierced/skinned cable or possibly it's not data grade cable. Once the phone started doing this, I moved it to a known good location and it continued the same behavior.
Performing the reset function as described above (in my case, we found that an external power cube was not required...works just fine doing it with a POE connection). This cleared what apparently was a corrupted load and the phone came up loaded, rebooted, and registered correctly.
So in addition the the reset, I would add that you should move the phone to a KNOWN GOOD connection when doing the reset. Customer had attempted this previously, but had not moved it to a known good location. Moving it made all the difference.

For me it just went from bad to worse...
7941 phones POE with bullseye boot.
Tried the 1 2 3 4 5 6 7 8 9 * 0 # reset - no luck. Rebooted - bullseye again, failed to register.
Next tried 3 4 9 1 6 7 2 8 5 0 * #. Both indicators flashed alternate red. After a few minutes it automatically rebooted it flashed alternate yellow for a few seconds then goes to a blank LCD screen not even a bullseye.
Phone appears to be "bricked" now....not happy.
