Caller-ID showing unknown on outbound calls through POTS

I am running CM 4.2 with an MGCP gateway at one of our remote sites. We have a POTS line setup. When one of our users dials a local number, the call shows up "unknown xxx-xxx-xxx" The number shows up fine.

I talked to the Telco twice and they said they have tested their side and the name of our business shows up when they test it. But this does not work when we dial from our side.

Does anyone know of anything that would block the name on our side from showing up on calls?

We called out from the FXS port and the name showed up correctly. But the IP phones are showing unknown.
