Installed lastest firmware (cmterm-78xx.12-7-1-0001-393.k3.cop.sgn) for Cisco 7821 to upgrade.
Firstly I pulled new firmware from the Sftp server to CUCM.
Then changed load firmware (from new to old )for all cisco 7821, cause don`t want to upgrade all phones except only one for testing.
Then only on one phone, inside "Phone Load Name" pasted new firmware name.
Saved, restarted Tftp service in CUCM serviceability.
Checked phone status, but shows "failed".

The problem was in CUCM SUB. We must add images to the CUCM SUB and PUB. We didn`t add images to the SUB.

try this one;
Download firmware image with ZIP extension
Extract files to a folder on your desktop
Upload all files (in that folder) to TFTP nodes (OS Admin --> TFTP File Management)
Restart TFTP services
apply sip78xx.12-7-1-0001-393 to 1 phone for test purposes.

@silvervoip did the same things but no success.
Also, I tried to upgrade 8851 got success but only one phone got success. Another 8851 get the same, Failed.

Please make sure;
-DB Sync is good between CUCM nodes
-Network is stable and you are applying WAN and LAN bandwidth requirements
-Latest Device Pack is installed. If not, please read release notes before applying.
what do you see on status message? you can check via web GUI
