FXO Disconnect issue

Hello All, 

I am facing issue with FXO line connection on my cisco 2911 series router on which i am running CME. 

Issue: My FXO lines are not getting disconnected even after users at receiver end or user who dial disconnect the call. 

Lines getting stuck in Off hook stage, every time i have to shut the port manually to make it work. 

I have contacted my ISP and they have provided me below frequency details, can anyone help me how would i design below frequency detail and applied on my router. 

Disconnect Tone Specifications:

 

X@-30,X@-30;Y(A/B/1+2) where:

X = frequency in Hz.

A = Time in seconds for the frequency.

B = Time in seconds for the silence.

400HZ@-30,400@-30;4(.375/.375/1+2)
425HZ@-30,425@-30;4(.375/.375/1+2) in case the above one does not work.

 

On-hook wink = 325 - 425 ms

Coin collect and operator released guard interval time = 2000 ms

 

Please find below attached frequency parameters which my ISP has asked me to configure for my FXO line.

Hi. I have the same problem with my provider. I guess we have the same provider. For me it is UAE du. I have even problems with incoming calls (50 % successful incoming call rate). Can you share part of your config regarding voice-ports?
