How would one check who deleted an extension on CUCM 6.1.4 ?

You can try using ausit logs. It records any changes in database. So you might get the extension deletion information there. Below is the link to configure that:

There are no audit logs in 6.X, you'll need to review the Tomcat logs to find this out.
