Inbound calls continue to ring even after handset is picked up

Hello,

We are having an issue with one of our locations. When an inbound call comes in, sometimes the call will continue to ring into the handset after the call is "answered" with the handset. Then our user can hang up the handset, the display will change to "unknown caller" and then is able to pickup the handset and answer the call. I have been working with our telephone service provider, but we use this same service provider at 2 other locations that are not having this issue. This also happens on all phones at this location, not specific to one phone.

Call flow: PSTN <-> FXO Port on Cube Router <-> CUCM <-> 8851 Phones

I've been working with TAC on this and we are unable to resolve this issue. The strange part is this issue only happens on one to two calls a day. There doesn't seem to be a link on the inbound caller, it happens on cell phone calls and landline calls.
