Loud Feedback Noise thru headset following call

When call center employee hangs up call from customer, a very loud noise is generated from the headset that causes severe ear pain. Sound is loud enough that co-workers can hear through the head-set. It has happened a few times but unable to reproduce circumstances. Here are the details around the phone and head set; Cisco Phone Issue - Crackling - D0C282D04A11 (phone ‘MAC ID”) Cisco IP Phone 7942 Headphones: Plantronics – S/N #0451PL. Further specific details include:

• We immediately put the phone out of service and then had it replaced the next day.
• The incident happened a few seconds after the customer call was completed and the customer had hung up.
• Employee says it happened immediately when he clicked the “End Call” call button on UD.
• Employee said that seconds after the sound the phone power cycled itself
• We have other, witnessed reports of a similar sounds happening to our phones when on ‘speaker’ mode (no headsets)

If anyone has experience with, please advise if there are any known issues that produce this noise and if solution is available.

I would try a different software version.
