I was playing around with the self provisioning and it all seems to work perfectly. There are a few cosmetic issues though. In my universal line template, I added variables #FirstName# #LastName# for line description and alerting name but for some reason those variable don't work. Yes, I've made sure my line template is assigned to the user profile used by that user.
Note - I'm trying to use self provisioning without LDAP. That means I create the extensions before hand and use the quick user add to create the new users.

I just figured it out. Do not create the extension prior to doing the quick add. During quick add, click the New button next to the extension drop down and fill in Directory Number and Line Template.

Thanks for letting us know that, I personally have only done this with LDAP and it's really easy as they're created at the same time the users sync, I haven't tried with local users.
