i have problem that incoming calls not appearing number for example if some one call me from his mobile to our number is showing unknown number so any call from out side is unknown ,please check my configurations :

Test and if it still does not work, remove all the config from the port and take a PCM. Share that.

Why do you have mode set to FSK and DTMF on the ports ? Just part of troubleshooting or is it necessary for your environment ?
If the added config did not fix the issue, just keep the following on your port and take a pcm -

why this happen?

Might be due to a CPU spike. You should not be running debugs along with PCM. HPI process can really spike up if the PCM are kept running. Just enable the PCM, reproduce the issue and stop the PCM immediately.

This is if you are running an IOS earlier than 15.2(2)T1.

If you are running a higher IOS, follow the following steps -

voice pcm capture buffer 500000
voice pcm capture destination flash:pcm.dat

test voice port x/x/x pcm-dump caplog fff duration xxx

Ensure that the "pcm.dat" file is 500 KB or higher. If not, you will need to collect the capture again.
