C9500-16X default to ROMMON mode

What would cause a C9500-16X (or any Cat9k) switch to default to the ROMMON configuration mode?

 

The switch itself had the IOS on the switch, it just required to be manually pushed through. During the day we had a fibre break, but this link was only responsible for resiliency purposes only, which doesn't explain how this switch defaulted to the ROMMON mode.

 

I suspect this to be a power related issue and an incorrect boot order, but I'd like to hear any person experiences.

Switch rebooted into ROMMMON?  Post the complete output to the following commands: 

  • sh version
  • sh boot
  • sh red
  • dir

Thanks for responding! I think I found my answer by following your commands. The switch was rebooted which explains how it got into that mode and there is a boot order, but only for the one switch. We have two switches stacked via SVL. I think I got it from here though, but thanks for the help!
