Is there a way to reset maintenance partition password or application partition password when you are locked out of both partitions?

Have you tried to boot into both application partitions?
cf:4 and cf:5 and you do not know the password?
In that is the case then yes, RMA is your only option.

Thank you for your quick answer. Unfortunately, the procedure does not help me, because I cannot authenticate to FWSM regardless of the boot partition (this being maintenance or application partition). I do not have credentials for any of those partitions. Is there any way out of this?

Hello,
Looks like the only way out of a situation like this one, is an RMA, at least this is how we managed to "solve" the issue, after we opened a Cisco TAC.
