Cisco.com has 3rd to 5th edition migration tools, but none for 4 to 5.
How is this done? What tool? Where obtained? What URL?

You don't need any kind of migration tools for upgrading from 4th to 5th Ed. You can directly upgrade from 4th to 5th Ed.
Refer to the section "Performing an Upgrade Installation->Upgrading from CD One, 4th Edition" in the Install Guide of 5th Ed for the procedure:
