Does this point to an issue with an SFP?
We have a 4500-x vss stack, through which runs the heartbeat for a bunch of vrrp groups upstream, both routers have been going active due to loss of heartbeat.
One of the switch uplinks has a tx power of -10db on the switch, but router upstream is showing healthy rx signal of -5db.

I dont think so. But, you can configure an IP address on those interface and ping to make sure there is connectivity and no pacekt loss.

We have the same setup at our company and had the same problem at our company.
Steps:
1) Check and clean fiber ends.
2) Try switching fiber cables or transceivers.
3) Check fiber patch panels.

Depends on the optics. If the optics are DOM-capable, it should show up in the logs if either one is reporting low power.
