Over the last 15 months I replaced 21 SFPs at one location. All were GLC-SX-MM. I recently started keeping track of the serial numbers.
The last 12 serial numbers started with FNS11363nnn. Could this be a bad production run of SFPs.

I recommend opening a case with Cisco TAC.
They need to look at the symptoms for each of the failures and check for any known issues with the Finisar SFPs.
When you open the case, list the exact symptoms for all the failures. If you opened Service Requests for the RMAs of the SFPs, list all the SR numbers so the TAC engineer has the history. List the serial numbers for each failure.
The TAC engineer will also need to trouble shoot the problem while there is a failed SFP in the node so they can collect logs, check PM, etc. I recommend waiting for another failure, then opening a Service Request so TAC can trouble shoot while the failure is present..
