Which CCM version are you using?? Which phoneload are you using on the 7912. May be its a bug. Have you tried upgrading it to a better one?

I have the problem only that I have a MCS7825 running CM, TFTP, DHCP all on one server. Let me know if it is really a bug.
Good luck

Hello,
I have a 7825 2.26 Ghz MCS with CM 3.3.4 and TFTP and DHCP on it. According to my findings, this is a combined problem that has to do with CM Server resources and the 7912 Software Load version. I solved this problem by doing the following:
1. Updated CallManager to 3.3.4 sr2
2. Removed all temporary shared drives from CallManager that might not be reconnecting at startup
3. Removed AutoAttendant to free some memory and startup processing.
4. Tuned TFTP Parameters to generate new configuration files for IP Phones (.cnf.xml) under Services --> TFTP --> Advanced
5. Disabled System Traces under Cisco CallManager Serviceability --> Trace --> Configure
6. Downgraded Load Version for 7912 and 7905 IP Phones to SCCP v.3.3.8
7. Manual Reset to Factory Defaults those IP Phones that showed its default Gateway as CallManager 2 SRST in the IP Phone network configuration .
8. Make sure to have the appropiate configuration for Auto-Registration (if used) under CallManager Group Option.
This changes helped me tune and solve this issue but your environment might be different than mine. So, do not take it as the unique procedure.
You can check CM's memory and processor utilization by Ctrl-Alt-Del and clicking in Task Manager --> Performance.
Good luck.
