Call Forward Issue

If A calls B and B has a Call Forward All to C, then C cannot transfer back to B (this functionality was available in the customer's legacy PBX). Is there a workaround?

Sounds to me like you want to have some boss/secretary setup. If so, you should look into IPMA, see:

I am guessing C is trying to transfer to B's voicemail, since all calls are forwarded otherwise.

What we did to overcome this was implement a blind "*" option to transfer all calls to voicemail. If a user in the cluster transfers a call to B's number, preceded by the * key it will route directly to their VM greeting.
