Is there a way to set ccm 4.0 so that if a call is on hold for over a set time, the call rings back on the line that put it on hold?

As of now, this feature is not available. But, a service parameter called "hold type" in CCM 4.1.
Hold Type : This parameter determines whether the handset lamp winks or flashes when a call is on hold. Valid values specify True (handset lamp flashes) or False (handset lamp winks).
This is a required field.
Default: false.
This is not exactly the same as what you are asking for, but this is one way of reminding the user who puts the call on hold that there is a call still on hold.

Thanks for the quick answer, I thought that would be the answer I would get. This is a feature that is needed ASAP, if you have a very busy office with multiple people answering the phones, it is very easy for someone to slip up and leave a call on hold for a very long tim.

Yes, I have seen a few requests for a similar feature and a feature request will be opened to get this feature available in later versions of CallManager.
