Call Recording Strangeness

I'm not looking for an exact answer, just some direction on what may have occurred.

We're running CCM 3.3(4) / Unity 4.0(3). One of my users just told me that she just received a voicemail of a phone conversation she had about a month ago. The conversation was between her and another party via PSTN. She was not at her desk, the phone was in a conference room. There wasn't no conference, just the two of them on the phone. Also, we do not have the 'on-demand recording' feature enabled.

Any insights are much appreciated.

In the Cisco Unity Assistant, the Cisco Unity Inbox, and ViewMail, subscribers can make and play recordingseither by using the phone, or by using the computer microphone and speakers and clicking the Media Master controls. Subscribers with the applicable class of service settings can also use the Media Master in the Cisco Unity Administrator to make and play recordings.
