when a phone fwds/all to another phone, the caller id does not appear on the ringing phone, display says fwd from xxx, where xxx is the phone that set fwd/all.
Is there a way to have call mgr send the caller id to the phone that is receiving the fwd'd call ?

In Callmanager 4.X, you can set this in the directory number configuration settings.
At the bottom of the page there is a section named Forwarded Call Information Display
Check the Caller Name and Caller Number and it will show the information for who is calling, not who forwarded the call. Below are the descriptions of all the fields.
Caller Name
Checking this check box will cause the caller name to display upon call forward.
Redirected Number
Checking this check box will cause the number that was redirected to display upon call forward.
Caller Number
Checking this check box will cause the caller number to display upon call forward.
Dialed Number
Checking this check box will cause the original dialed number to display upon call forward.

I have the same problem and the above solution doesn't work. When the phone ringing it display the forward party phone number.
This apply to IPCC as well, instead of displaying the original calling number, it display the CTI Port number that are used to forward call to agent.

IPCC is not forwarding a call it is actually transferring the call so you are going to see the IPCC port because that's what it's using to place the call to the user and then transfer the call once it's connected.

checked off the caller number box and now the caller number shows on the display bottom of the ringing phone. fwd'ing info still shows on the top of the display.

I might not have mentioned this before:
Uncheck the Dialed number field and the Caller Number should show up at the top.
